Pool Forms

Below is a list of all the necessary pool documents that need to be signed and returned before you will be allowed access to the pool area this Summer.

Documents to be printed, completed and returned

General liability release
Emergency contact info (hard copy print out needed)
Disciplinary guidelines (including children’s “signatures” and info)

These forms are required for us to be covered from liability. Studdiford has the right to refuse entry to the pool area to families that have not completed and returned these forms. Thank you for your cooperation.

THE RULES AND POOL TIMES HAVE CHANGED. PLEASE REVIEW THE NEW RULES AND TIMES.
